Orthodontic coverage is usually separate from routine dental coverage. The plan may apply a percentage to eligible treatment up to a lifetime maximum and may include age or dependency rules.
A treatment plan or predetermination can show the proposed procedure, fee schedule and expected insurer contribution. Payment may be made to the patient or provider, in instalments or according to the insurer's rules. Coverage can change when employment, plan year or dependent status changes.
Ask for the orthodontic percentage, lifetime maximum, amount already used, age limits, waiting periods, preauthorization requirements, payment assignment rules and treatment-in-progress provisions before signing the financial agreement.

The Prepare stage turns interest into a plan. A meaningful consultation should connect the clinical exam, photographs, digital scans and any necessary radiographs to a written treatment recommendation. It should also explain responsibilities, monitoring, refinements, retention, insurance and what happens if the original plan changes.
A predetermination is not a guarantee of payment. It is based on current eligibility and information, and treatment decisions should not be presented as insurer decisions.
Many plans apply the same orthodontic benefit category, but the insurer and policy wording control the answer.
The April 2026 CDCP guide says a specified range of limited orthodontic services may be requested at a date still to be determined, with preauthorization and clinical criteria. Do not assume elective Invisalign is covered.
Ask both insurers about treatment-in-progress rules and coordinate with the clinic before coverage ends or changes.
